MFA Failure Procedure

1. Verify Root Cause: Ensure the failure isn’t caused by simple Clock Drift (check NTP sync) or a depleted battery in a hardware token.
2. Obtain Master Admin Credentials: Retrieve the physical “Break-Glass” envelope from the Captain’s safe.
3. Activate Console Access: Connect locally to the OT system/Gateway (MFA is usually bypassed on local serial/VGA console ports).
4. Log Incident: Complete the authorization section below before making any system changes.
5. Notify CSO: Inform the shore-side Cyber Security Officer within 4 hours of the bypass.
