Phase Conclusion: Restoration & Resilience
The Recover phase ensures the vessel returns to a “safe, consistent, and known state” (UR E26 §4.5.3). By completing this phase, the ETO has proven that the ship can survive a total system compromise and emerge with improved defenses.
Audit Evidence Checklist (Recover)
Before closing this phase, ensure the following evidence is ready for DNV inspection:
- Verified Golden Images: Master clones for all Category II and III systems, stored offline and encrypted.
- Restoration Verification Logs: Quarterly “Dry Run” records proving that a full system wipe and restore works as intended.
- Incident Response Documentation (§4.4.1): Detailed logs of any recovery actions taken, including software versions and integrity checks.

You have successfully navigated through all five functional phases of the IACS UR E26 framework.
Next Action for ETO: Present the “Master Audit Summary” to the Master and Chief Engineer for formal sign-off in the ship’s Safety Management System (SMS).
